 USE THIS WORKSHEET to organize yourself and know what you need to do!  Know your deadlines! (regular, merit, scholarship, etc)  Letters of recommendation!  Request transcripts!  Send SAT/ACT/AP scores!

 A Transcript is an official copy of a students academic record.  It includes the following: ◦ Course names from 9 th -11 th grade ◦ Final grade for each course ◦ Senior courses ◦ Weighted AND Unweighted GPA and Class Rank – after Oct 1st **** No SAT, PSAT, HSA, ACT, or AP scores will be included.

You will need to complete/update the following:  Submit your COMPLETED BLUE transcript request form to the Student Services Secretary at least 4 weeks prior to your college application deadlines (REMEMBER HOLIDAYS/BREAKS).  $4 for each transcript requested  Complete FERPA Waiver on CommonApp/Naviance - COMPLETE BY SEPTEMBER 15 TH.  Official transcripts for college/universities accepting electronic transcripts will be sent by your counselor.  Official transcripts for schools that DO NOT ACCEPT electronic transcripts, will be mailed by the Student Services office.

 Make sure you put the exact deadline date, for each school, on the request form VERY IMPORTANT!!!  When a college will accept the Common Application or their own application, MAKE SURE you indicate which application you are using on Naviance.  NCAA – fill out Transcript Request Form, $4  Ms. Deison will be available to take your Transcript Request Forms:  7:10 to 7:20 every morning  During all lunch shifts Monday through Thursday

 COLLEGES/UNIVERSITIES MAY REQUIRE A TEACHER EVALUATION FORM OR A SCHOOL COUNSELOR FORM to be filled out and accompany the written recommendation. DON’T WORRY – teachers and counselors will send this form (and the recommendation) ELECTRONICALLY. We will print out the form and send it to schools who do not accept electronic submissions.

 Some colleges require letters of recommendation from one or more teachers. FOLLOW THE DIRECTIONS that the college has stated about the selection of teachers and number of letters needed. ***NAVIANCE WILL ONLY ALLOW THE SELECT NUMBER OF TEACHERS TO SEND REC’S ONLINE!  Provide any updated information or changes to your resume on Naviance – for teachers and counselors!  Follow up with a thank you note to your teachers and counselor for writing your recommendations!  *Teachers will keep the request forms for their own records.

TEACHER RECOMMENDATION REQUEST FORM 1.Include ONLY THE COLLEGES that you want that specific teacher to send recommendations to!!! 2.Include application deadlines. 3.Mailed or Electronic? 4.Is it a school for which you are using the Common App? 5.Make sure you sign and a parent/guardian sign!! 6.A new Data Form will need to be completed for additional schools after the teacher receives the initial Data Form. 7. GIVE THE DATA FORM TO THE TEACHERS at least 4 weeks before the college application deadline. REMEMBER HOLIDAYS/BREAKS!

 IN ORDER TO SEND TRANSCRIPTS AND RECOMMENDATIONS ELECTRONICALLY, you must sign the FERPA Waiver on your Common App account, THEN also connect your Common APP account and Naviance. YOU MUST DO THIS IF YOU ARE USING COMMON APP FOR ONE OR MORE APPLICATIONS.   If you are not using COMMON APP at all, you do not need to do this.

 Create Common Application Account at  Complete Education section  Add at least one college to yourMy Colleges tab using the CollegeSearch tab  Under the My Colleges tab, click on one college (on left hand side), the click on Recommenders and FERPA. Fill this out, the information will be used for all schools to which you are applying using Common App.  Go back to Naviance, Colleges Tab, Colleges I am Applying to, match you Common App and Naviance accounts by following the directions.

 You received your unofficial transcript today. See your counselor if you have any missing or incorrect information on your transcript.  Official Class Rank and GPA’s will be available after October 1 st !

 MRHS COLLEGE VISITS – list on Naviance!!!  SIGN UP FOR VISIT RIGHT ON NAVIANCE!  YOU WILL GET AN REMINDING YOU OF THE VISIT 24 HOURS AHEAD. PRINT THE AND USE IT AS YOUR PASS (SHOW TO TEACHER) TO ATTEND THE VISIT!  ATTENDANCE WILL BE TAKEN AT EVERY VISIT AND FORWARDED TO THE STAFF.

IMPORTANT!!!! If all of your paperwork was turned in on time, we will submit our paperwork, and then it is up to the college/university to upload the paperwork on their end. When the college uploads the transcripts/recommendations is up to them. Also, the college/university has to then update their system (what you see online), and sometimes that can take a few weeks.

If you want to have an individual conference with your Counselor, please come and sign up in the Student Services office! If you need a Counselor Recommendation, you must meet with your Counselor!!!!
