COUNSELOR RECOMMENDATION- Complete candidate biography form and/or resume and return to your counselor with your waiver signed by student and parent (if under 18) TRANSCRIPT RELEASE FORM- complete and return to your counselor- $2/school Cash or Check payable to HKHS ♦ Colleges have different deadlines- you must follow each college’s deadline’s exactly as they dictate. ♦ SAT I & II scores- check with your college if they will accept scores off the transcript or if you need to send them from College Board. ♦ Application & fees vary per school.

School Reports/ Records Q: What is sent from the school? A: A: ♦ Official High School Transcript (1 st page consisting of all courses and grades, 2 nd page listing all school related clubs, organizations, and sports, and a check list to see the student has met all graduation requirements) ♦ SAT/ACT test scores (provided your student indicated our school code upon registration and we have a sticker from the College Board and ACT.) ♦ Teacher Letter of Recommendation (1) ♦ Counselor Letter of Recommendation (1) ♦ Secondary School Report (including student’s senior schedule, class rank, and QPA) ♦ School Profile Q: How will we know if the college/university has received our materials? A: When all materials are sent out, we send a postcard with each application packet that is then signed by an admissions officer upon receipt and returned to us.
