Making The Community Connection At Howard Community College

What is The Community Connection Program? A Community Inclusive “Transition Program”

The Community Connection Program A community based post high school program Located on the Howard Community College campus, Designed for students with mild to moderate disabilities Who are in their last years of public school education.

“Characteristics” of potential students

Student Characteristics 19 years of age will receive a Maryland HS Certificate, not a Diploma minimum of four years in a High School setting demonstrating the ability to function independently with minimal supervision. IEP goals and Transition plans are appropriate demonstrates satisfactory attendance and behavior

Referral to The Community Connection Program is based on an IEP Team recommendation * All students will be reviewed on a case by case basis
