About Community Opportunities Community Opportunities is a local agency who helps people, living in Lincoln County, who have developmental disabilities (such as autism, cerebral palsy, seizure disorders and mental retardation).

Our Promise to You You can choose to have Community Opportunities or the state-Hannibal Regional Office-provide case management to help you. If you choose Community Opportunities, we promise we will listen to you and help you plan for the supports you need. There is currently no waiting list for Case Management Services.
There is a cost for Case Management but it is paid by either your Missouri HealthNet coverage or by Lincoln County. You will not have to pay for Case Management services.
Our staff are all Qualified Developmental Disability Professionals and have many years of experience helping people.
We will help you with: - Writing your own Individual Support Plan - Having it clearly explained to you - Having it clearly explained to you - Saying what you want in the plan
We will accommodate you. If you need reasonable accommodations such as an interpreter or help getting into and out of a building, please let us know and we will do everything we can to assist you.
What will Community Opportunities Case Management offer me? An active role planning in all of the services you receive. Help in finding community resources. Checking the services you receive to make sure they are doing what they are supposed to do. Help with paperwork. Responding to you in a timely manner. Advocacy assistance.
