Goals Project: Be a leader in ambulatory chronic illness care Improve process for providing & managing a quality focused primary care practice Provide & educate community on importance of health care performance information to make active decisions about their health care-prioritizing diabetes care first - - QUALITY OF CARE and SELF-MANAGEMENT Consumer Engagement: Provide easy to understand information on what quality of care is as well as provide accessible information on hospital & practice data Educate patients on disease self-management & physician/patient relationship building to ensure high quality results

Consumer Insights Diagnosis –surprise/inevitability, fear, denial, feeling overwhelmed Living with diabetes –stigma/shame –food/diet daily challenges –crave social support & info Diabetes care received –inconsistent self care; poorly informed about Rx. –Unique challenges for un/underinsured: prioritizing Rx/medical care and extending time between MD visits Quality –Defined as MD who listens, spends time with me, shows caring by how they communicate (e.g. eye contact) Standard of Care Guidelines –No awareness –High level of interest in MD guidelines and self care checklist NCQA/ADA Physician Recognition –No awareness –MD/patient relationship more important

Diabetes Footprints Campaign Knowledge is Power The key to success with diabetes is in your hands. The more you know, the better equipped you are to take charge of your diabetes. For Caring For Yourself: A checklist of things you should monitor at home. An explanation of why these things are important. When you should and shouldnt be concerned. Diabetes Footprints Website for local resources including: –Classes for newly diagnosed –Nutritionists –Recipes and Meal plans –Support Groups –Link to the local ADA For Doctors Visits: A checklist of tests recommended by the ADA, that doctors should do on a regular basis. A list of questions to ask your doctor, including topics such as medications, treatment options, what to expect physically and emotionally, diet & exercise. A simple explanation of the meaning of the numbers on your test results.
