Must bring the following documents to our office: 1) Memorandum requesting visa 2) Visa Application as required by country 3) Valid Official Passport for all military or DOD employees 3) Tourist Passport (Contractors only) 4) Visa Pictures (Will be taken in our office as required) Note: If you are from outside USSOUTHCOM, you must mail documents to: Passport Office, ATTN: Mr. Torres, 9301 NW 33RD Street, Doral, FL 33172. Also, you must include a prepaid Airbill so we can return your passport UNCLASSIFIED
