Alberta Health Services- Aspen Update October 21, 2008

. Mandate: to improve coordination within the health system so that it is easier to access and to ensure all Albertans have the access to the care they need when they need it, regardless of where they live.

Continuum of Care Division Goal #1: To create a system where those receiving health services and supports can move as smoothly and seamlessly as possible between different areas of the health system, and that access is improved.

Goal #2 To ensure effective and efficient delivery of supports to keep people healthy, in the areas of health promotion, injury and illness prevention, screening and early detection, vaccinations, home visits, nutritional counseling, home care, health information and more.
