The purpose of this RFP is to select a vendor that can satisfy the State’s need for Janitorial Services at multiple locations. – Indiana State Government Center Complex Government Center North Building Government Center South Building (includes Department of Workforce Development) Indiana State Library – IDOA-Logistics Service Center – Indiana School for the Deaf – Indiana School for the Blind and Visually Impaired – Indiana Veterans Home, West Lafayette, Indiana – Department of Revenue, Ameriplex Industrial Park Purpose of the RFP

Scope of Work (Section 1.4 of RFP) Attachments I-M for scope of work for each location. In the future, the State may require Janitorial Services for additional buildings or offices throughout the State. It is understood that each building/office has a unique set of cleaning requirements due to size, number/type of floors, layout, amount of furniture/equipment, etc, but the pricing proposed during the RFP process will assist in completing cost comparisons when future location services are required.

Scope of Work (Section 1.4 of RFP) The Indiana State Library and the IDOA Logistics Service Center are currently being cleaned by a State Use vendor as a subcontractor being managed by the incumbent. RFP requires all respondents to utilize an approved State Use vendor as a subcontractor at these two locations. The subcontractor shall provide all of the janitorial staff for the locations, except for any supervisors/managers. Please see Attachment O for a list of State Use vendors that provide Janitorial Services.

Term of Contract The term of the contract shall be for a period of two (2) years from the date of contract execution. There may be two (2) one (1) year renewals for a total of four (4) years at the State’s option.

Business Proposal (Attachment E) Company Financial Information (Section 2.3.3) – Confidential information must be kept separate from the proposal in both hard and soft copy Contract Terms (Section 2.3.5) – Respondent should review sample State contract (Attachment B) and note exceptions to State mandatory and non-mandatory clauses

Technical Proposal (Attachment F) Please use the template provided. Where appropriate, supporting documentation may be referenced by a page and paragraph number. Any supporting documentation should be identified in Attachment G, Summary of Attachments.

Proposal Preparation Buy Indiana, Business Proposal (2.3.13) – Status shall be finalized by proposal due date – 5 definitions, details provided in Business Proposal – Indiana Economic Impact, Attachment C – Definition of FTE (Full-Time Equivalent), Example The state defines FTE as a measurement of an employee's productivity on a specific project or contract. An FTE of 1 would mean that there is one worker fully engaged on a project. If there are two employees each spending 1/2 of their working time on a project that would also equal 1 FTE.

Proposal Preparation Please return all attachments in the format provided. For example, Cost Proposal (Attachment D) must be returned in Excel format. Use the templates provided for all answers. Do not alter templates.

Minority and Women’s Business Enterprises Prime Contractors must ensure that the proposed subcontractors meet the following criteria: Must be listed on the IDOA Directory of Certified Firms Each firm may only serve as once classification – MBE or WBE A Prime Contractor who is an MBE or WBE must meet subcontractor goals by using other listed certified firms. Certified Prime Contractors cannot count their own workforce or companies to meet this requirement. Must serve a commercially useful function. The firm must serve a value-added purpose on the engagement.
