Facility Name  Conducted by ADD WHO  Physiologic Assessments must occur quarterly w/ CC schedule  Bio Sway/Balance Trainer SD Assessments occur quarterly as well (All facility residents are considered)  Discuss Who Analyzes Results and what happens w/ them  Discuss Recommendations  Refer to the CV website for the “How To”videos (up in August 2010) OutReach  Conducted by ADD WHO  Physiologic as well as Bio Sway/Balance Trainer SD are to be offered every 3 months  Right to refuse participation Just make sure to document what occurred  Discuss Who Analyzes, Recommends and Shares w/ participant  Refer to the Care Ventures Website for the videos
