Premises:  Comprehensive, coordinated, family- centered care that takes advantage of community resources will improve outcomes for children, especially those with special health care needs

Premises (cont.) :  Maintaining up-to-date knowledge of the evaluation and treatment of children with chronic and complex conditions, and available community resources, is difficult for primary care clinicians

Premises (cont.) :  Partnering between clinicians and families will leverage their expertise, time, and knowledge and will be enhanced by sharing information.

Just another day at the office (in Utah and, hopefully soon, beyond)…

Ramone, your five-year old patient with Prader-Willi syndrome, vomited twice this morning but doesn't really look sick. Should we see him today?

The health department called about Jessica, a 5-day old with a positive newborn screen for methylmalonic acidemia. What do we do now?

Your first patient today, a 5 year old, is new to the practice, coming in for a well child check and the schedule says he has fragile X. Is there anything special you should know or do?

Juanita, is here with her son who was recently diagnosed with autism. Having read a lot of confusing stuff on the web, she wants to know how to find reliable information about the diagnosis, planning for his schooling, and how to help his siblings deal with this.

After a long hospital stay, Tiffany just went home with a new tracheostomy and the home care company wants orders and a letter to support the need for an in-line humidifier.
