11 Explanation to Participants about this template and final presentation This template is meant as a Six Sigma / Re engineering Submission Template This template gives guidelines. The Paper is expected to have evidence that the project was carried out using the principles of the six sigma /re engineering.

33 The Charter should cover the following elements: A. The Business Case: The rational behind initiating the Six Sigma / Re engineering project from the business perspective. This part should answer why the project is important to the business and if it is addressing Short/Medium or long term challenges/Objectives. Project Charter

4 B. The Problem Statement: The description of the issue the way the team stated it in the project. C. Process mapping and how it was done. D. Identify Critical to Quality and to Process Indicators, please indicate how these indicators were recognized. 4

5 Project Charter E. The Goal Statement. Description of the set objective. F. The teams involved: Please indicate team and Organizational roles. G. Stakeholders model: Describe the relations and interests. H. The project timelines. Need to describe original set time limits for the various parts of the project vs actual completion dates. I. Quick Win opportunities (If any) 5

6 Measure A. Measurement Plan listing and description plus the rational behind it. B. Measurement system Analysis for the above if needed Gauge R&R or measurement system limitations or reliability C. Determine Process Capability 6

7 Analyze A. Problem Statement Identification of factors contributing to the issue/problem. This is done through data stratification methods B. Validation of the Root Cause: Please indicate how this was carried out utilizing tools such as Design of Experiments or any other Statistical Methods. 7

8 Improve A. How were potential solution alternatives developed? B. Indicate Solution Criteria that was set by the team C. Show Evidence of Solution Evaluation D. Show Solution Selection Process (For Example this could be the ranking method) E. How was implementation planned, carried out and monitored? F. Process performance before and after implementation G. Improvement results – Cost saving/ revenue addition results verified by Finance deptt. H. Intangible improvements, if any C.. 8

9 Control A. Controls put in place to ensure that the gains are locked in and that the process is stable. (Describe the control system elements) B. Describe the roles of the stakeholders in this process C. How long the stability of the process was monitored? What was the result of monitoring (e.g. process capability, control chart limits etc.). 9

10 Project Closure A. Learnings from the project B. Where there any horizontal deployment opportunities. C. Plan for further refinement of the process D. Team recognition. Indicate how the team was recognized. Plan Do Check Action
