It is with great pleasure that NYCOM has joined with SSP Honorary Osteopathic Service Fraternity and we hope to make this the MOST successful chapter in the Nation! You have been chosen based on academic as well as service excellence! Be proud, you are now an OMEGA! Our first meeting is forthcoming and our induction ceremony will be held in the Spring. Please review and complete the attached forms and bring them to the meeting. Be advised that as a member of SSP you will be required to complete a specific set of requirements to maintain your SSP status. Sincerely, SSP Selection Committee

Omega Chapter- SSP Acceptance form Please initial and sign below. ___ I agree that as an active member of the Omega Chapter of Sigma Sigma Phi, I will: Attend regular meetings, with a limited number of unexcused absences as deemed by the Omega Chapter Executive Board (currently two unexcused absences allowed). Maintain an academic average of at least 80% with no failures. Coordinate/chair at least one SSP event during an academic school year. Complete at least 3 hours of community service per term Report all community service hours and chaired events to the chapter Secretary after each event. Attend the Annual Induction Banquet held in the spring of each year. Pay Chapter Dues of $75 (a one-time fee). ___ I understand that failure to fulfill the obligations set forth by the Omega Chapter is grounds for dismissal from Sigma Sigma Phi as well as a letter of dismissal sent to ERAS.
