Also please make sure that you attach a copy of their source document ( EX: school schedule, pay stub, volunteer letter, Military orders). At the end of each report please submit your comments and observations of that months activities and any information you need for us to know about. If no contact was made please reflect that on your MMR. You will still be required to submit a report even if no contact was made.
