SEN Assessment Team Responsibilities Manage referrals for statutory assessment Monitor Quality of Annual Reviews & Pupil Progress Amend statements every key stage or 3 years unless significant changes Attend Independent/Non Maintained reviews every 2 years as a minimum in consultation and co-ordination with Social Care Support transitions through: –Attendance at all Year 5 annual reviews –Attendance at all Year 11 reviews –Attendance at all Year 14 reviews
