Preparing your child’s Health Portfolio n Immunizations n Physical Exam n Dental Exam n Professional Eye Exam n Emergency/Health Form n Birth Certificate

Immunizations n The State of Wisconsin Requirements: n 4 DTP/DTap/Td (one dose must be after the 4th Birthday) n 4 Polio n 2 MMR (first dose must be after 1st Birthday) n 3 Hepatitis B n 2 Varicella (Chickenpox) n Waivers are available for religious, health or personal conviction reasons.

Physical Exam n Your child will need a physical exam. n Form must be completed and signed by the physician or nurse completing the exam. n Make appointment now, as clinic schedules fill quickly.

Dental Exam n Not required by the State of Wisconsin. n BUT.. n Encouraged by our District. n Protect your child’s SMILE!

Professional Eye Exam n Recommended by the State of Wisconsin. n Separate form in 5 year old Kindergarten packet.

Emergency/Health Form n Complete initial form. n If your work, cell or home phone number changes, notify the school immediately. n Review Student Information form on a yearly basis, revise as necessary.

Birth Certificate nUnUsed to verify age of child, and proper spelling of student’s name. nNnNo copies will be kept on file at school.

Medication n Proper form needs to be on file. n Prescription medication is a Physician’s order, not a parent’s request. n Pharmacy filled, properly labeled bottles only. n DO NOT allow children to carry medication on their person. n NO COUGH DROPS!

Illness n Please keep ill children home. n First Aid is provided by the school nurse or office secretary. n Report all communicable diseases. n Teach your children good hand washing Soap, Water and Friction

Breakfast n Every morning should start with a good breakfast. n Breakfast is available at school. n Engines run better on premium fuel!
