Federal Work Study Instructions in Completing the Forms Student Financial Services

I-9 Form  The Purpose of this form is to document that each employee is authorized to work in the United States.  Please complete the highlighted section (Section 1) on the form completely.  A photo ID, such as a drivers’ license OR a Purchase College ID (called the MORE Card) AND your social security card OR a certified and original copy of your U.S. birth certificate is required.  As an alternative, an unexpired U.S. passport will satisfy both requirements for documentation.  Please note that job placement will not occur until these forms are handed in.

W-4 Form  The Purpose of this form is for employers to withhold the correct federal income tax.  Line 5: Please put “0” if you are a dependent of your parents and will only have 1 job. (This applies to the majority of students)  Line 7: Please write “exempt” if the following statements are correct.  Please do not forget to sign and date the form.  For most students, it is not necessary to complete the second page  Please note that job placement will not occur until these forms are handed in.

Direct Deposit Form  Please complete Section A and B completely.  Please attach a voided check to this form when submitting it to the Payroll office.  You may fill out this form anytime during the academic year. Please keep in mind that it will take time to process. Section C  Only fill out this section if you are using a savings account or a checking account without providing a voided check.  Your financial institution must fill out and complete this section. Section D  Please do not forget to sign and date this form.  If you hold a joint account, it is mandatory to have your joint account holder sign the form also.  Please return this form to the Payroll office.

Payroll Schedule  Please keep in mind the deadline dates.  The financial aid office will need 1 day to process the timesheets and therefore all timesheets should be handed in to Financial Aid a day before the Payroll deadline. Dates highlighted in Yellow.  Your office may pick up the checks or you may do so yourself with your MORE card from the Payroll Office. (Please check with your office to see what procedures there are for payroll pick up.) dates highlighted in Pink.
