Purdue Healthcare Advisors Founded in 2005 as a non-profit initiative by the Purdue Technical Assistance Program, the Regenstrief Center for Healthcare Engineering, and the Indiana Hospital Association. What is H3? Healthy Hearts in the Heartland Federally funded research program by the Agency for Healthcare Research and Quality (AHRQ) awarded to Northwestern University Focus on small practice clinics in Indiana, Illinois, and Wisconsin Determine the best ways to support small practices with quality improvement

How does H3 work? Receive 1 year hands-on coaching Tools and strategies to provide point of care support for Million Hearts cardiovascular measures (ABCS) Randomized practices will also receive support in population health management Collect and analyze clinical quality measures Implement and evaluate quality improvement strategies for cardiovascular care

Who can participate? Small Adult Primary Care practices with <10 or fewer providers Must have certified electronic health record technology Do not currently receive significant quality improvement support Located in the northern half of Indiana (including Indianapolis)
