Incident Reporting

Incident Reporting Our vision is an environmentally safe and incident-free workplace. We are committed to providing a safe work environment for everyone. Incident reports and investigations will be completed for ALL injuries, accidents, property and/or product damage and serious near misses.

Incident Flow Chart Contact the physician hotline Employee notifies supervisor Branch verbally notifies EHS Drug & Alcohol test completed Incident report form completed by end of shift Incident report sent to EHS within 24 hours Incident investigation is completed within 24 hours Contact the physician hotline before transporting patient or when reasonable to do so. If there is an injury EHS can waive drug & alcohol testing for non-fault incidents

Responsibilities What are the responsibilities of an employee involved in a work-related incident? Ask this question to the group. Prompt a discuss on what they believe the responsibilities are. Report the incident immediately to his or her supervisor Seek medical attention as soon as practical Complete the Incident Report Form Ensure branch management and EHS is kept informed of current conditions. Cooperate in any investigation that may follow the incident Not place themselves or others at risk of injury

Notification What type of incidents need to be reported to the on-duty supervisor? Ask the group what they think is reportable. After a group discussion, reiterate that ALL incidents must be reported regardless of how minor. All injuries (even the very minor ones) All accidents with potential for injury Property and/or product damage situations All “Near Misses” where there was potential for serious injury

Injury Notification If the incident involves an injury, HealthForce must be called. If the injury is life threating or needs immediate attention, call 911 and notify HealthForce when it’s possible to do so.

Notification – All incidents EHS must be notified immediately following notification to the supervisor/manager. The notification to EHS can wait if emergency medical attention is needed Drug & Alcohol testing will be completed immediately following an incident or within a reasonable time frame.

Incident Report A written report must be submitted to EHS within 24 hours of the incident. All vehicles are equipped with camera kits. Take as many photos as possible of damaged equipment, other parties damaged property and the surrounding area. Employee(s) involved in the incident will give a written statement by the end of their scheduled work day on the date of its occurrence.

Incident Review A thorough investigation will be conducted which may include an incident review. Facts will be collected and analyzed to determine the cause of the incident. Employees are expected to participate in any review for an incident they are involved in. The level of review will depend on the severity of the incident (i.e. some may be held at the local level while more severe incident reviews may include EHS participation).

What’s the risk? 1 out of every 25 employees was injured in 2013. We are committed to sending each employee home safely each and every day. We want them to return to their families in the same condition they came to work in.
