Small, everyday choices can seem unimportant. But they add up. And ultimately they can affect you and your family’s health safety and wellbeing. So choose better health. Choose to keep your family and your community safe. Choose better training and education. Because when you choose a better future here, this community has your back.

Community Service Navigators Goal- to connect citizens with health & human services that will increase access to opportunities & choices that promote health, safety & wellbeing Target population- citizens with multiple or complex needs who are struggling to get by & have significant barriers to accessing health or social services to address these needs

Enhancing Choices for Our Citizens Safety, health & wellbeing start within our families, schools & neighborhoods Many citizens can’t afford medical care or have difficulty locating or getting to health or social service providers Everyone should have the opportunity to make choices that lead to health, safety & wellbeing

Enhancing Choices for Our Citizens Social factors like income, education & ethnicity mean some people are more vulnerable to poor safety & health outcomes Rural residents have fewer options for services and/or challenges in identifying & accessing resources
