After you Sign Up… Four Things: 1.Pick health insurance plan through Michigan Enrolls 2.Pick a primary care provider (PCP) - Doctor 3.Schedule an appointment with your PCP (Doctor) (must be within 60 days of picking your health insurance) 4.Complete the Healthy Michigan Plan Health Risk Assessment form

1 st Step: Pick a Health Insurance Plan

2 nd Step: Pick a Primary Care Provider Also known as your DOCTOR s for Doctor? Primary Care – the first point of contact a person has with the health system; where people receive most of their everyday care. It includes the prevention, diagnosis, treatment and follow-up of various health conditions.
