Tracking and monitoring are essential to ensure the success of the plan. In performing such duties, the case manager must be able to: Maintain the participant/ case manager relationship Keep the client focused on the goals of the plan Make it clear the plan will be monitored Tracking / Monitoring the IDP Maintain the business/ case manager relationship

Sample Consent to Release of Information Letter I, (client’s name, address and phone number) hereby give my consent to the release of (state the specific information that should be released) from (specific name of the organization that will be releasing the information) to (specific name of the person to whom information will be released, include name, address and phone number). The release is effective from (state specific date) to (state specific date). The purpose for the release of this information is (state the specific reason for which the information). This consent may be revoked by me in writing at any time, any information released prior to such revocation remains released in accordance with this consent. I have read and understand all of the above; all blanks have been filled in prior to my signature. This consent was freely and voluntarily given.
