What is Patient-Centred Care?

“Granny Video” Improving the patient experience identified as a key priority for HSN. ‘Granny Video’ - an excellent demonstration of what it means to be – or not to be – patient centred.

Patient-Centred Care There are a number of key initiatives underway at HSN in which patients and families are a key component of our planning efforts. These include: – Quality Improvement Plan – Organizational Excellence – Patient/Family Advisory Councils at the program level – Patient representation on: Board committees (ie. Quality Committee) Selection committees (ie. Executive recruitment)

CEO Patient and Family Advisory Council The experience of care, as perceived by the patient and family, is an important factor in health care quality and safety. Purpose of council is to provide advice on how we can deliver quality, safe and compassionate care at HSN that is first and foremost centred on patients.
