Division of Population Health Sciences Royal College of Surgeons in Ireland Coláiste Ríoga na Máinleá in Éirinn Diagnostic accuracy of the STRATIFY clinical prediction rule: a systematic review and meta-analysis Jennifer Billington, Rose Galvin, Tom Fahey

Division of Population Health Sciences Outline Background Methodology Results –All studies –Sensitivity analysis Discussion

Division of Population Health Sciences Background

Division of Population Health Sciences Background Falls are a significant cause of morbidity and mortality and frequently lead to lasting loss of mobility, fractures and limitations in social participation Inpatient cost of fall-related hospitalisations among older people is currently estimated at €59 million 32% of incident report forms in UK

Division of Population Health Sciences STRATIFY CPR St. Thomas Risk Assessment Tool In Falling elderly –Derived in 1997 in the UK Consists of five items –past history of falling, patient agitation, visual impairment affecting everyday function, need for frequent toileting, and transfer ability and mobility –The STRATIFY rule yields a possible score between 0 and 5 (each item scoring 1 if present or 0 if absent) –≥2 points – high risk of falls Several validation studies exist

Division of Population Health Sciences Study aim Peform a comprehensive systematic review and meta-analysis of validation studies of the STRATIFY CPR to determine its value as a prediction rule for identifying patients at high risk of falling.

Division of Population Health Sciences Methodology

Division of Population Health Sciences Methods PRISMA guidelines Search strategy –Pubmed, EMBASE, EBSCO, Science Direct, CINAHL and Cochrane library –Limit: 1997 – 2011 Data extraction –Study setting –Patient demographics (age, gender) –Population type (e.g. geriatric rehabilitation) –Number of falls/no. of individuals who sustained a fall

Division of Population Health Sciences Methods Inclusion criteria: 1.Validate STRATIFY CPR 2.Prospective or retrospective cohort studies 3.Setting: hospital, nursing home, community based 4.Studies that record a subsequent fall 5.Sufficient data to calculate –Sensitivity/specificity –Negative and positive predictive values

Division of Population Health Sciences Methods Quality assessment (QUADAS) –Risk of spectrum bias –No definition of fall provided Meta-analysis: Bivariate random effects model –Sensitivity analysis Studies which did not define a ‘fall’ excluded Studies with evidence of spectrum bias excluded

Division of Population Health Sciences Results

Study identification Two researchers (JB,RG) screened all articles 1,571 articles identified 16 studies with 17 separate cohorts included (n=11,378)

Division of Population Health Sciences Discussion

Division of Population Health Sciences Discussion The diagnostic accuracy of the STRATIFY rule is limited at the cut point of ≥2 The CPR should not be used in isolation for identifying individuals at high risk of falls in clinical practice Sensitivity analysis - broadly comparable results Results in keeping with previous systematic review –sensitivity 0.67 (95% CI ), specificity 0.51 (95% CI ).

Division of Population Health Sciences Conclusions This study pools data from a broad range of studies and settings, enhancing the generalisability of its findings This systematic review shows that it is more useful to rule out falls in patients who score <2 Essential to establish the diagnostic accuracy of falls prevention tools identify patients at risk of falling more accurately We suggest that STRATIFY CPR may be used as part of a multifactorial falls prevention strategy
