Overview  Definitions  Activity Life Cycle Phases  Tools  Lab Project

Definitions Hazard “Any actual or potential condition that can cause injury, illness, or death, damage to or loss of equipment, property or degradation of organizational goals.” (TSI) Risk “A combination of the severity of the mishap and the probability that it will occur.” (MIL- STD-882E)

Definitions Probability “An expression of the likelihood of occurrence of a mishap.” (MIL-STD-882E) Severity “The magnitude of potential consequences of a mishap which include: death, injury, occupational illness, damage to or loss of equipment or property, damage to the environment, or monetary loss.” (MIL-STD- 882E)

Activity Life Cycle Phases (Identify, Analyze, and Control Hazards)

Risk and Hazard Tools What are risk assessment tools?  Mechanism for the evaluation of common operational factors and their associated hazards (Operations Phase)

Risk and Hazard Tools Concept and Planning Phases

PHL and PHA  The Preliminary Hazard List (PHL) is just what it sounds like, a list. Simply put, it is a brain storming tool used to identify initial safety issues early on in an activity concept phase.  The Preliminary Hazard Analysis (PHA)  PHL, PHA: What’s the difference?  What is needed to perform an effective hazard analysis?
