Patient Safety Learning Networks (PSLN) Project Overview ( ) Purpose: Recruit and support hospital teams to learn how to use AHRQ-funded tools and resources to improve patient safety Tools and Resources Offered  Hospital Consumer Assessment of Healthcare Providers and Systems (using HCAHPS to measure and improve the patient experience of care, and reduce readmissions)  Medication Reconciliation toolkit (reduce ADEs and readmissions)  Project RED (to reduce readmissions)  Door to Doc (improve ED flow management)  Quality Indicators (identify hospital adverse events) PSLNs sought to create ongoing collaborative relationships between hospitals 2
