Section A – February 1 st CAO Form. March 1 st Tick the ‘Opt into DARE’ box. SIF Form filled out by student on CAO. All require an ‘Evidence of Disability Form’ to be filled out by relevant Consultants / Occupational Therapists etc. Section B – April 1 st Impact Statement filled out by school and the student. Student sends to CAO. Section C – April 1 st Evidence of Disability Form. Student sends to CAO.

Section C – April 1 st Copy of Psychological Report. Summary Sheet as filled out by Psychologist. Can submit Evidence of Disability Form and Psychological report if necessary.
