4.Major Incidents Summary 6 DateLocationDetailsFailure analysis To Characteriza tion LAB Precision A/C tripping problem solved with Vendor. Blower touching the body of the precision unit and got tripped continuously to LT panel room Power fluctuation at LT panel room every 30 minutes. Due setting problem in main panel control system, it was tripping. Solved with the help of vendor Clean room DI water leaked out and empty the DI water tank totally. DI water inlet to wet bunch aspirator was open position and drain valve not opened UtilityPCW flow meter damaged. Actuator valve stem damaged. Due to pressure increased in the PCW system flow meter damaged.

5.Initiatives taken –Old clean room Panels and tube lights dismantled – Diffusion room flow meter changed – Characterization lab precision A/C PPM done – SF-30 A/C drain line leakage problem solved – FF-10 Pedestrian fan repaired – Compressor shifted form Old clean room to utility building – TF -31 power socket alternation works done –SF-9,SF-10, FF-9 Light fittings fixed & panel board cleaned LT Panel room tripping problem solved with vendor – Characterization precision A/C tripping problem solved with Vendor – SF-10,Sf-11 Connection changed from raw power to UPS as requested by AN murthy – House vacuum PPM done – FF-10 Ceiling Fan fixed – Old clean room partition dismantling done – CeNSE building drinking water sensor problem solved – TF-11,21,10,12 UPS power provided as requested by Manjunath – SF Amps socket fixed – Clean room DI water leakage due to improper operation of wet bunch aspirator and drain valve solved Damaged PCW flow dry etch room replaced – SF-16,FF-03,TF-21 UPS connection provided as requested by Narahari. Characterization lighting problem solved out – TF-32 earthling problem solved.

6.Initiatives Planned for next month 8 Full fill of manpower. Training for maintenance staff. CeNSE building electrical room PPM. LT room PPM. CeNSE Building chilled water refilling & chiller repairing.

13.Suggestions and Requirements 15 Shelters to be provided for DGs & Fire pump house to avoid rain water and leaves come inside the DG cabin & Raw water tank in side. Shelter with closed fencing can make on the Fire tank & this area use as storage place. PC with internet connection and telephone to be provided for maintenance team. Uniform & Safety shoes to be provided for all maintenance staffs. Walky-talky to provided for maintenance staff. CeNSE building – drawings and other documents to be handed over properly which is related with FM team. DG-3 (CeNSE Building) B-Check pending.
