Completing the Form AFFIDAVIT OF SERVICE FORM 08A

OVERVIEW With this template, you will be able to fill out an Affidavit of Service for the Plaintiff’s Claim, Notice of Default Judgement, Notice of Garnishee, Examination, etc. Re-use this form each time you are to fill out a separate Affidavit of Service. After you have completed the form, print as many copies as you wish. If there are multiple plaintiffs or defendants, the party that is being served must receive a copy of the Additional Parties 01A form as well. COMPLETING THE FORM

On the Affidavit of Service (FORM 08A) change your FORM STATUS by clicking once on the white box. A dropdown menu will appear. Select ‘Active’ from the list, which means you are currently working on the document. Once you have completed the data entry at the end of this process, you’ll change the status to ‘Done’. If you would like more information about the FORM STATUS move your cursor over the help prompt in Column ‘F5’. Form Status COMPLETING THE FORM

Affidavit Tupe COMPLETING THE FORM Documents Affidavit Type: Select the document requiring an Affidavit of Service. (ie.,Plaintiff’s Claim, Notice of Default Judgement, Notice of Garnishee or Examination, etc.) You will need to use this 08A Affidavit of Service Template for each required form. Each one should be printed out individually once it is completed.

Page 1 COMPLETING THE FORM Rows contain all the information that has been transferred from the 07A form. You will not need to re-enter any information that you have previously filled out in other forms. If there are any additional plaintiffs or defendants, the Additional Parties 01A form must be filled. You can access this form in the MENU index.

Enter the full name of the person initiating the service (plaintiff or defendant) in ‘C 62’. Enter the municipality or province of residence of the person initiating the service in ‘C 63’. The full name of the person/company who is being served will be automatically be filled in ‘C 64’. Enter the date of service in ‘C 65’. Page 1 COMPLETING THE FORM

Update Debtor COMPLETING THE FORM Select the name of your defendant for Defendant No 1. in E 64. Click on the Update Debtor button, and that defendant’s information will be transferred to rows 67.

COMPLETING THE FORM Rows : Read through the form and mark a small or large ‘X’ in the dropdown menu for any specifics that apply to your claim. In the areas where there is no dropdown menu, type out the required information.

Form Status COMPLETING THE FORM Once you have finished filling out the 08A form, review the information and make sure it is correct. Scroll up to the top of the form and change the form status to ‘Done’ (if you feel the form is ready and complete). If it is still pending, you can change your status to ‘Pending’ and come back to it if you need to add or change any information.

Generate & Merge COMPLETING THE FORM Click on and the form will be generated into a Word document that opens up automatically in M-S Word.

A pop-up dialogue box will appear, prompting you to run an SQL script. Click ‘Yes’ to continue. SQL Script COMPLETING THE FORM

1.Two Word documents will appear. ‘Form Letters 1’ is the document you need. Behind it is the merge document SCR_08A, which you do not need. Please close the merge document without changing it. 2.Review the ‘Form Letters 1’ document carefully to see if all your information has been generated correctly. If you wish, use Save As to save the output where you want to store it. 3.Print as many copies as you need of the document. You can now bring these along with you to the Small Claims Court.
