Patient-Centered Medical Home

What is a Patient-Centered Medical Home? It is an efficient approach to health care. It means you and your doctor are the leaders of your medical home team assisted by: nurse practitioners, physician assistants, nurses, medical office staff and other health care professionals. In some ways, you will not notice any changes but in other ways, you will see a new focus on meeting your unique needs as a patient. So, whether you are a new patient or have been coming to name of clinic for a while…welcome to your Patient-Centered Medical Home!

What will the Patient-Centered Medical Home do for you?  It will help you receive health care when and where you need it.  If specialist care is needed, it will help you get to the right specialists with the correct information  It will help you and your health care team keep track of your health information  It will help you get to know and feel comfortable with your health care team

How can you help? Tell your medical home team about any other health care professionals who care for you:  Specialists  Dentists  Therapists  Eye doctors  Pharmacies  Foot doctors

 Any new medicine  Any medicine you are no longer taking  Any vitamins or supplements  Any over-the-counter medicines  Don’t forget about eye or ear drops, inhalers, and any creams or lotions you are using Help your team to keep an up-to-date medicine list in your records every time you visit. Remember to tell your medical home team about:

You can help your medical home team by:  Telling your medical home team what your goals are for your health  Telling your team how you feel about the care you are getting from them, what has worked for you in the past, or what hasn’t worked  Knowing what steps you can take to improve your health before your next office visit  Asking for needed refills of your medications before you leave the office

Why are we talking about Patient–Centered Medical Home ? The current health care system can be confusing. You may visit more than one doctor and feel lost in the system. Sometimes, you may even have extra medical tests or appointments because of seeing different doctors. The medical home team will efficiently take care of you, listen to you, help you keep track of your health information, and help you find your way through the system
