Communities Putting Prevention to Work King County’s CDC Obesity Prevention Grant ($15.5M for 2 years). Implement evidence-based policy, systems, and environmental changes. Sector-based changes: Schools, local government, community-based orgs. Make healthy choices easier; increase opportunity for physical activity and access to healthy food.

Five Conditions of CI 1. Common Agenda: A shared vision for social change. 2. Shared Measurement System: A short list of key indicators across all participating organizations. 3. Mutually Reinforcing Activities: A diverse set of stakeholders, typically across sectors. 4. Continuous Communication: Frequent communications to build trust and inform ongoing learning and adaptation of strategy. 5. Backbone Organization: Ongoing support provided by independent staff.
