The Tri-Hospital Health Equity Data Collection Project This project aims to develop a model for hospitals to gather patient demographic data to improve quality of care and patient outcomes Celebrate Data !

Why we want to collect Socio-demographic data To identify health disparities by linking patient health data to patient socio-demographic data To determine: – who we are serving – whether health needs are being met equitably –if there are differential outcomes To improve the quality of care in our hospitals and ensure excellent care for all Data is good for your health!

What we hope to learn Which socio-demographic data to collect What questions to ask and how to ask them The most effective ways to gather sensitive personal information IT solutions that will integrate the collection of socio-demographic patient data into hospitals systems Data Now! More than Ever!

Objectives To determine the key demographic characteristics of the patients we are serving To have better data to measure patient health outcomes and health inequities To develop and pilot an effective data collection methodology To evaluate health equity outcomes based on clinical indicators To ensure knowledge exchange of successful methodologies to other institutions Our Data is our Strength!
