Records and Documentation: Databases Survey questions and answers for the 4th Conference of Ombuds and Deans of Students in Higher Education Australasia.

Introduction The intent is to share the resulting information to examine what common ground and applications we have. What information is kept, how and why

1. What databases do you employ or prefer? Preferred databases vary from university to university – most common Access Others use combinations of applications for different reasons: For keeping documents Access, Word, Excel others do not use databases for keeping documents. Keeping track of complaints/grievances Access; Lotus Notes; Filemaker Pro; Word and Excel

1. What databases do you employ or prefer? Tracking problems Access; Lotus Notes; Word, and Excel Keeping track of curriculum, academic, program or transfer Calista, Excel, or Access, however, many do not use data bases for this purpose.

2. How long do you retain/store collected information? Broad range of responses from: data being retained only until the problem is solved to keeping data for 5 years, 7 years, 10 years, or in some cases, in perpetuity.

Do you follow Australian complaints standards regarding complaints, or more specifically those of your particular university? 1 respondent follows the Australian complaints standards majority appear to follow their particular university policies 1 respondent stated a preference for creating policies based upon other benchmark models.

Complaints in respect to discrimination/ conduct for either staff or students: The majority indicated no, to recording certain details initially, In some cases respondent’s details are recorded after the accusation is ‘substantiated’ – though not clear motivation Confidentiality of the respondent being protected; the majority said yes Note - not all respondents have the role of discriminatory complaints management, but are the final person referred to should complaints

5. Regarding records dealing with complaints, are they compared or scrutinised to see if any obvious problematic trends are apparent, either regarding staff or students? Why? Why Not? For most universities this seemed to be a must, although at least one did clearly assert that no records were kept to compare. The reasons for comparison were to facilitate change and to examine trends or problem areas. “part of the role of the student ombudsman is to encourage and facilitate improvements in the university’s administrative and academic quality and so records are scrutinised to determine if particular areas have common problems, or if there are generic issues that trouble the students”

6. With the information you collate and store… At least half use information else where - with restrictions on how and with whom. The majority use it in part or whole for examination, or reporting, as well as for analysing trends. Only a few respondents stated they use data to enable them to be proactive

6. With the information you collate and store… Most respondents had hard copy storage as well as computer storage the protection of hard copies was stressed by many as being a serious priority. On this issue of whether others analyse the data, investigate, question or follow up with further action, the majority of respondents indicated ‘no’.

7. How do you analyse the data collected? What ‘type’ of data are you collecting and why for example: Analysis and collection of data seems to be a problematic area: 1/3 of respondents indicated Date of complaint/grievance, The majority do not indicate sex or gender The Program/Course was minimally noted The nature of the complaint/grievance minimally noted

Questions raised in records and documentation survey Are university policies examined annually regarding possible updates, amendments etc? What is the response time is regarding action taking place on a complaint, and the resolution time? Does anyone follow up long term ( months) after an issue, problem or complaint has been discovered/made and successfully dealt with?

(Discrimination etc.) Is it possible that many complaints never get reported for various reasons, ie, complainant fear of reprisal, or fear of negativity from others, ie peers or relevant staff? Some organisations have created a report sheet for such events where complainants would prefer not to be identified, while the report is acknowledged no action is pursued, but most importantly the data is stored and available for analysis should other events or complaints or problems match. Is this sort of solution a possible consideration for some areas of sensitive complaint at university level?
