Antacids How do they work? O E.g. CaCO 3 + HCl  CaCl 2 + H 2 O + CO 2 O E.g. magnesium hydroxide, calcium carbonate

Alginates Mechanism of action: O Upon mixing with water they form a viscous “raft” layer that sits on top of the gastric content, protecting the oesophagus from reflux O E.g. sodium alginate (Gaviscon)

Drugs that protect the mucosa Bismuth Chelate O Coats the mucosa O Enhances prostaglandin synthesis O Stimulates HCO3 Misoprostol (PGE 1 agonist) O Binds the PGE 1 R on parietal cells O Inhibits HCl secretion O Increases mucosal blood flow O Augments mucus secretion Misoprostol causes uterine contraction – contraindicated in women of child bearing age

Proton Pump Inhibitors How do they work? O Antagonise the H + /K + ATPase pump preventing acid secretion into the stomach lumen O E.g. omeprazole, lansoprazole

H 2 receptor antagonists How do they work? O Prevent histamine binding at H 2 receptors, causing a knock on decrease in acid secretion O Eg cimetidine, ranitidine Cimetidine is a liver enzyme inhibitor (important in polypharmacy)
