State Mandates Before children enter Kindergarten they must have state mandated immunizations, which are: 1. 4 doses of Polio 2. 5 doses of DTP 3. 2 doses of MMR 4. 3 doses of Hepatitis B 5. 1 dose of Varicella or have had Chickenpox’s
State Mandates Physical examinations and TB tests are also necessary for entering kindergarten and should be done no more than six months before school starts to meet both the Kindergarten & First Grade physical requirements.

Medical Conditions It is very important to share any medical conditions (ex. allergies, asthma, diabetes, or history of seizures) that your child may have with the health office prior to starting school. Together we can develop a plan for your child to make the transition to school easier.
Medication We prefer that all medications be given at home. If your child needs medicine (prescription or non prescription) during school hours, this can be done with a form that requires a doctor’s order and parents permission. Forms can be found on the IUSD website under Health Service. Physicians orders must match the prescription bottle and medication must be delivered to school by the parent in the original container.
Emergency Information Please complete the emergency card and list all possible phone numbers where you can be reached in case of an emergency. You will be called if your child is ill or injured. If your child vomits or has a fever above 100 they will be sent home and need to be fever and symptom free for 24 hours before returning to school.
Healthy Habits It is recommended that kindergarten students have hours of sleep and eat breakfast before coming to school. Please send your child to school with a healthy snack. Brush teeth twice a day Wash hands before eating and after using the restroom
