Intent To ensure coordination of caregiver services in Wisconsin counties, Aging Units

Older American’s Act Programs 3 Area Agencies on Aging 72 counties – GWAAR provides technical service for 70

The launch of the grant program Counties under spending III-E dollars Creative idea to offer RFC (Request for Collaboration) to establish or sustain a Caregiver Coalition

Grant Process and Allocation GWAAR released a RFC for WI Aging Units to create or sustain a Caregiver Coalition (see Appendix 1) 2009 – 10 grantees - $46, – 15 grantees - $58, – 4 grantees – $16,200

Coalition Support Statewide coalition grantees networking meeting held in July 2010 Another round of grants

Caregiver Coalition Development 2012 GWAAR designated an employee to work with the new coalitions Advisory Group formed to share best practices and projects Information compiled was shared at four NFCSP regional trainings

Caregiver Coalitions – Best Practices and Projects Presentation and discussion of the document (see Appendix 2) Sharing among other newly-formed coalitions Development of a communication mechanism

Share The Care (STC) Stations Four of the Caregiver Coalitions have started a STC Station as a project Share The Care - a caregiver support model Coalition members are trained as Station Managers 6 STC Stations in Wisconsin

Green Co. Caregiver Coalition 2011 Grantee ( NAC 2012 scholarship recipient ) County originally had a “Dementia Care Network.” ADRC responded to RFC, to blend partners into one caregiving coalition Contracted with the local Alzheimer’s & Dementia Alliance for staff support

Mission: To support, educate and advocate for those who give and receive care. Initial goals: Community survey & focus group Success: 20 agencies/organizations, community volunteers and family caregivers. Challenges: To keep each individual/agency engaged and involved
