Review Wiki Comments… 11 Paul de Raeve (European Federation of Nurses) – At EU level, we deploy 4 levels in acute care: Healthcare assistants Registered nurses (complying with Directive 2005/36/EC) Specialist nurses Advanced Nurse Practitioner – These 4 levels has been developed by the ICN and the OECD Member States are aware of these developments. – So, I would not use the term ""medical assistant"" as this is extremely confusing – For the Registered nurses, Art 31 sets out the competences (8) – It is important to reflect the nursing care continuum and they all 4 need to have different levels of IT skills We will look at this when we talk about roles and role mapping to ensure that we make the correct levels of nursing care mapping between the EU and US

Comments CONTINUED 12 Paul de Raeve (European Federation of Nurses) – From a nursing perspective, the levels indicated in the excel file (from 1 to 8) do not fit the reality within the European Union. In nursing care, there are different level of qualification but there is no split between clinician and non-clinician and this division can create confusion. From a nursing perspective, as mentioned previously, there are 4 levels of qualifications/occupations, that are not reflecting completely the basic, intermediate, advanced and expert levels. The 4 categories of the nursing care are: healthcare assistant, registered nurse, specialist nurse and advanced nurse practitioner. All levels work in the acute care setting and their skills level is “cumulative”, meaning that there is no need to create new professions. The EFN is willing to work on skills mix and skills needs of the 4 categories. Additionally, in relation to quality and safety, the dynamics and the staffing ratio between the different levels should be taken into consideration and analysed.

Comments CONTINUED 13 Stuart Werner (U.S. Department of Labor Employment and Training Administration) – Electronic Health Records (EHR) Competency Model: – The model is depicted in a graphic consisting of several tiers. The arrangement of the tiers in a pyramidal shape is not meant to be hierarchical, or to imply that competencies at the top are at a higher level of skill. The model’s shape represents the increasing specialization and specificity in the application of skills as you move up the tiers. Tiers 1-5 have been developed and are divided into blocks. The blocks represent competency areas, that is, the applied skills, knowledge, abilities essential to successful performance in the increasingly electronic environment of the health industry. Comments CONTINUED 14 Stuart Werner (U.S. Department of Labor Employment and Training Administration) – A Joint Letter of Support from the Assistant Secretaries of the Employment and Training Administration, the Office of Vocational and Adult Education, and the Administration for Children and Families encourages states and local areas to align resources that support integrated service delivery across Federal and state funding streams and calls for improved collaboration and coordination to support career pathways systems for youth and adults. – Career Pathways Webinar Series: _Series_Availabl/info _Series_Availabl/info – Career Pathways Toolkit: – Virtual Career Network – care.aspx care.aspx An online tool that will allow current and prospective health care workers to explore more than 80 different occupations, identify local education and training programs, and tap into current job listings. Through the Virtual Career Network, users also can learn how previous experience such as military or on- the-job training can be applied toward a health care career, take free online courses and find sources of financial aid. We will explore these sites and offerings when we begin our exploration and gap analysis of what exists to ensure competencies are met

Comments CONTINUED 15 Anne Moen (University of Oslo, Norway) – I do think that it would be useful to think about more of the clinical roles that can be involved in advanced care, ex. physical therapy, (Advanced Clinical-Level 6) – AND think along two dimensions of work; "1:1" interactions of some kind that should be documented and "1:many" which involves a lot of interdisciplinary coordination work, The latter could be added to this skill level – I find the suggested skill levels nice and congratulate you on the effort so far. One general comment is that the skill levels seems quite hierarchical and it is somewhat difficult to see more of the interdisciplinary collaboration and coordination that also spans the levels in the suggested hierarchy. I believe these aspects ought to be accommodated for to ensure HIT support for smooth operation
