MHS ORCHESTRA ACTIVTY PARENT & UNIFORM ORDER FORM Full Name: __________________________________ Grade: ____ Orchestra: ________________ LADIES UNIFORM: All ladies will wear the “Concerto” dress. They are comfortable, don’t wrinkle easily, and are machine washable – cutting out the cost and time for dry cleaning! Students will have chance to try on the dresses at the Highlander Kickoff on September 1st, as well as during the first week of school. This is NOT the same dress that was used at Longfellow Middle School! LADIES FOOTWEAR: Black dress shoes must be purchased independently from a retailer of your chose. Close- toed shoes are preferable. Black hose or stockings must be worn with the uniform. ************************************************************* GENTLEMEN’S UNIFORM: All levels of orchestra will be wearing tuxedo pants, shirt and a cummerbund that is ordered through the orchestra. Tuxedo pants and Tux shirts from the previous year or from Longfellow MS are acceptable (if they still fit). All freshmen or new students will be loaned a button covers and cummerbund by the orchestra. If you are a returning orchestra gentleman, your uniform should be complete – unless you have grown over the summer! Check now! All gentlemen are loaned a tuxedo jacket and garment bag on a yearly basis. The loan of cummerbund, tux and button cover are included in your MHSO activity fee. Students are responsible for keeping the jacket free of damage and stains while it is in their use. The replacement fee for the jacket is $56, for the garment bag is $6 and for the cummerbund is $8. By signing the back of this form you are accepting this responsibility. If a jacket, garment, or cummerbund are lost or damaged the above amount will be assessed and must be paid in full before you graduate/leave MHS. GENTLEMEN’S FOOTWEAR: Black dress shoes and BLACK SOCKS (required for performances) must be purchased independently at a retailer of your choice. ************************************************************* All Students: REQUIRED ACTIVITY Payment: Please note that we are collecting a $50 activity fee per student to cover general program expenses such as music and instrument purchases and other consumable items allowed by FCPS. This will allow us to apply 100% of the profits of our student fundraising events towards sectional coaches, trips, professional development, etc. Please include the activity fee in the check for uniform rental/purchase costs.
