Future Forum & Integrated Care Services How can we take advantage of the health and social care modernisation programme to ensure services are better integrated around people's needs? What does good look like? Where can commissioning be better integrated for users & carers? How can integrated services achieve our objectives of better health, better care and better vfm? What, if any, barriers should be removed? Who needs to do what next and at which levels? How can innovation in integrated care be nurtured?
Adult social care engagement Caring for our future: Shared ambitions for care and support Quality: improve quality of care and support workforce? Personalisation: give people more choice and control over their care and support, making informed decisions? Shaping local care services: a wide range of organisations that provide innovative and responsive care services and that respond to people’s needs and choices? Prevention: support effective prevention and early intervention to keep people independent and in good? Integration : in partnership with Future Forum The role of financial services: what role can the sector play in supporting users, carers & families?
