The Project Roadmap Where have we got to? Where are we going?

The Malnutrition Task Force Independent group of experts across health, social care and local government, united to address the problem of preventable malnutrition in older people

Malnutrition Task Force Mission To ensure the prevention and treatment of malnutrition is embedded in all care and community support services as well as raising awareness amongst older people and their families

Developed five principles for best practice ①Raising awareness ②Working together ③Identifying malnutrition ④Personalised care, support and treatment ⑤Monitoring and evaluation

Phase 1 Research, workshops, learning examples and case studies Understanding the issues Understanding what looks good Finding out what interventions have taken place to tackle malnutrition before being clear on what has worked

The Five Pilot Sites The pilot areas are: –Gateshead –Salford –Dorset –Kent –Lambeth-Southwark

Implementation Working together they have been commissioned to support the pilot sites Coach in each site to guide and support the project
