CHANGE (Community Health Assessment and Group Evaluation) Tool

Modules Is there a policy, guideline or protocol to guide or influence behavior? What supplemental evidence and observations can we gather?

Physical Activity: To what extent does the work site provide access to onsite fitness center, gymnasium, or physical activity classes? Policy: An employee wellness plan that includes physical activity has been developed and adopted. Flex time policy for physical activity. Environment: Is there a fitness center or a place for employees to be physically active? How easy is it for employees to use this space? Do they use the space?

To what extent does the work site provide access to an onsite fitness center, gymnasium, or physical activity classes?

To what extent does the work site provide a safe area outside to walk or be physically active? Policy: Plans approved for walking trails, building plans for gyms, maintenance policies, flex time policy for physical activity. Environment: easy accessible, and safe trails, benches, lights are provided, equipment, and surroundings in good condition.

Nutrition: To what extent does the work site institute healthy food and beverage options in onsite cafeteria? Policy: Does the worksite have a RD that follows USDA guidelines. Environment: Does the cafeteria at the work site offer healthy menu and beverage options such as salad and veggies first and upfront, and milk and fruit juices. How easy is it to access healthy foods and beverages? Are there posters of healthy foods?

To what extent does the worksite offer healthy food and beverage options in the onsite cafeteria?

Nutrition: To what extent does the worksite provide land, money, pavilion for community- wide farmer’s market or community garden. Policy: Has the agency’s board entered into a joint-use policy or MOU. Environment: Farmer’s markets are available onsite; community plots are cultivated onsite; funding to sponsor community gardens or farmer’s market.

T obacco: Does the work site provide health insurance for tobacco cessation services, quitting aids or implement a referral system? Policy: Does the work site have an employee health insurance plan that includes tobacco cessation; is the plan approved by the agency; is the health insurance plan re-assessed? Environment: Is the insurance policy reviewed with the employee and is there an opportunity for the employee to provide feedback on the policy; is the insurance provided at a reasonable rate?

Chronic Disease Management: To what extent does the work site adopt an emergency response plan? Policy: Is there a written emergency response plan? Are there written safety training procedures? Has the plan been adopted? Environment: Are employees aware of the plan and trained to implement it? Are there emergency equipment and drills/table top exercises?

To what extent does the work site adopt an emergency response plan?

Leadership: Does the site participate in the public policy process to highlight the need for community change to address chronic disease and related risk factors? Policy: Is there a position description that includes participation in the public policy process to address chronic disease and related risk factors, are there agency policies that support such activities, are there position papers? Environment: do employees attend township meetings, county commissioner meetings or city council meetings to ask for needed community change to address chronic disease?
