US Procedures for TB Patients Originating in the U.S. and Traveling to Mexico YES NO  Educate the patient about the Card.  Ask about patient’s travel plans.  Distribute Card to patient. Should patient receive Binational Health Card?  Patient not from Pilot Site  Active TB patient with  1 month of treatment left YES NO  Record patient name, date of birth, Card number, date Card given, and reason Card given in logbook.  Transmit this information by fax ( ) to CureTB every 2 weeks.  Register patient as official/unofficial U.S. TB patient and assign RVCT# or “fake” RVCT#.  Register patient as “Binational”. Patient will travel to (or has left for) Mexico? YES NO  Patient has active TB and meets inclusion criteria [1] Does patient originate in El Paso/Las Cruces (including INS detention Ctr.)? Will patient move within the U.S.? Call TBNet ( ) and complete referral forms. [2] TBNet will call CureTB and fax the referral forms. Call CureTB and complete and fax referral forms [2] to CureTB. If the patient has not left yet, provide 2 weeks of TB medication prior to departure. As appropriate, continue asking about travel plans and educate patient about Binational Health Card. Call both CureTB and TBNet. YES NO END CureTB will inform the UIES [3] (800#) in Mexico. [1] Inclusion criteria: At least one of the following: Mexican-born; recently arrived to the U.S. from Mexico; migrant worker; close or immediate family lives in Mexico; works in the U.S. and lives in Mexico; or lives in the U.S. but receives medical care in Mexico. [2] Appendix B: Binational Notification Referral Form Appendix C: Supplemental Referral Form I, Appendix D: Supplemental Referral Form II (if not an official U.S. case) [3] UIES = Unidad de Inteligencia Epidemiológica y Sanitaria FAX CureTB: FAX UIES:

U.S. Procedures for TB Patients Originating in Mexico and Traveling to the U.S. HCW attempts to locate patient. HCW receives a call from CureTB about the arrival of a patient. Patient with TB Card Found? NO HCW continues to attempt to locate patient. YES Patient found. Patient is not found, call CureTB ( ) for more information. END  Call CureTB to notify of patient’s arrival and provide information on Card status (i.e., arrived with Card: Yes/No) and Date of Arrival.  Register patient into TIMS (use fake RVCT number if necessary).  Register patient as “binational” using binational variable (in sites using this variable).  Send all referral forms (Appendices B, C, and D) [1] to CureTB.  Report final treatment outcome to CureTB who will communicate this information to the UIES [2] (800# ) in Mexico.  CureTB will call HCW every 3 months to ensure final treatment outcomes are determined.
