Welcome to the Bentworth Middle School Health & Physical Education Program. Below you will find the policies and procedures for the 2014 – 2015 school year.

 Locker rooms are available for use, and students will be required to change clothes for physical education class. Students will be required to change out of their school clothes and into gym attire at the beginning of class, and back into school clothes at the end of class. Students are NOT permitted to wear gym clothes under their school clothes.  Appropriate attire includes:  Athletic shoes that tie or Velcro (Shoes w/ laces must be tied)  NO slip-ons or backless shoes  Any athletic shorts, sweatpants, warm-up pants  Any t-shirt or sweatshirt  NO jeans, jean shorts, boots, crocs, clogs, flip flops, sandals, dress shoes  PE attire must follow school dress code

Note: Depending on the weather conditions, students may be going outdoors for PE class, so we recommend bringing clothes to keep students comfortable in various temperatures.

 Students will have the opportunity to earn up to 5 points per class period.  2 points - Being prepared with and changing into and out of appropriate PE attire each day  1 point - Putting forth maximum effort in class activities  1 point - Demonstrating full participation throughout the entire class period  1 point - Following all Gym rules and class procedures

 *Note: Students who do not change clothes will still be required to participate in class activities (with a two point deduction) as long as a safe and supportive athletic shoe is worn.

 O (outstanding) = 90%-100%  S (satisfactory) = 70%-89%  N (needs improvement) = 60%-69%  U (unsatisfactory/failing) = 0%-59%

Health class points will be added in to make one grade for PE/Health. Students will receive a grade for PE/Health on their first semester report card and end of the school year report card.

 *Note: An alternate, cognitive assignment will be given to students who are physically unable to participate in class due to illness, injury, not dressed appropriately, or choosing not to participate. This assignment is given so that all students have the opportunity to learn the concept of each activity being taught in class. This assignment is NOT to be viewed as a punishment, but as an opportunity to learn. Students are encouraged to complete this assignment in class.

 If a student is unable to participate in the physical activities planned in class, parents/guardians should provide a written excuse. If an excuse is NOT submitted and the student does not participate in class activities, points will be deducted from the student’s grade. If a valid excuse IS submitted, the student will not be required to change clothes or participate in physical activity.

 Excuse from home: please include the date, student’s first and last name, reason for not participating, a parent/guardian signature, and a phone number for possible verification.  A maximum of FIVE excuses from home will be accepted in the course of one semester(2 nine weeks). If the student is still not able to participate in class after five excuses from home have been submitted, a doctor’s excuse will be required.

 Doctor’s Excuse: please specify the nature of the excuse and the dates that the student is to be excused from activity. Students will only be permitted to participate in class again when released by the doctor, and written documentation of that release is provided to the teacher.

 CELL PHONES, I-PODS, AND ANY OTHER ELECTRONIC DEVICES ARE NOT PERMITTED IN THE LOCKER ROOM OR PHYSICAL EDUCATION CLASS AT ANY TIME!!!  These devices must be left in the student’s school locker, and are NOT to be brought to PE class at all. This rule is strictly enforced for the privacy and protection of our students!!!

 Come straight to locker room at the beginning of class – wait in hall until Mr. Shrader or Mrs. Henderson tells you it is ok to come in.  The only time you do not come straight to the locker room, is when we are on a shortened schedule for the day – 2- hour delays, early dismissals, etc. Class will always meet, but will be shortened – so in that case, come straight to the gym (you do not need to change, but you do need sneakers).  Always wait outside the gym doors until Mr. Shrader or Mrs. Henderson says it’s ok to come in.  Choose any locker in the locker room – it’s yours for this period ONLY – take all things with you when you leave.  Change clothes as quickly and as quietly as possible (3-4 minutes).  NO ONE is permitted to change in the bathroom stalls!

 Put all items IN THE LOCKERS – do not leave anything on the bench or on the floor.  NO ONE is permitted in the shower area AT ALL.  Three at a time are permitted around the corner to the bathroom stalls and the sink. Use this area for restroom use and hand washing only!  Always ask before coming into PE teacher’s office.  When ready for class, sit quietly on the bench in front of your locker until everyone is ready and teacher takes you to the gym together.  Use restroom and get drinks while in the locker room BEFORE heading out to the gym!  Do not leave at the end of class until teacher dismisses you (EVEN IF THE BELL RINGS!)

 At the beginning of class, report directly to your squad line and have a seat  DO NOT TOUCH any equipment that may be out in the gym until directed to do so!  Remain in squad line (even if excused or not taking gym that day) until after teacher takes roll.  When whistle blows, you are to FREEZE! Stop all movement and talk and listen to teacher instruction.  When the long whistle blows, indicating that class is over, put all equipment back where it belongs and line up on the black line, at the door – GIRLS on the right, BOYS on the left.  Remain in a QUIET, straight line, and proceed into the locker room in an orderly fashion.  Gym rules and rules against bullying are posted on the bulletin board in the gym

 FIRE DRILL – stop what you are doing, drop equipment, and line up at the front door QUIETLY! We will proceed outside to the basketball court and line up on the far side. SILENCE is required during a fire drill. Secondary exit is the back door in the gym.  If there is ever a fire drill while we are still in the locker rooms – get yourself dressed as quickly as possible, and line up at the door. We will proceed to the back exit (near the cafeteria) and onto the softball field.  SEVERE WEATHER DRILL – line up at the back door, proceed to the locker rooms, assume the “tuck and duck” position on the floor, facing the lockers.  LOCK DOWN DRILL – line up at the back door, proceed into the locker rooms, sit on the bench (away from the door) and remain quiet! The locker room is the safest place to be!!!
