A mobile website application designed to enhance the quality of healthcare received by older adults on-the-go 1

Team Introductions All students are from Arizona State University Team Leader – Dan Weberg RN, PhD student in the School of Nursing and Health Innovation Team Members – Kris Anderson, Sophomore in Computer Science – Tim Exposito, Junior in the School of Sustainability – Gabrielle Palermo, Sophomore in Biomedical Engineering – Susanna Young, Senior in Mechanical Engineering Note: The team created this site without the expertise of a web developer using open-source resources such as Drupal, Google, and HHS/Medicare. All work was done by the team members listed above.

Problem Older adults migrate to Arizona to get away from colder climates. In doing so they may leave behind: Medical records Familiar doctors Vital documents Comfort of local healthcare SNAP Health was built to address the issue of lack of web-based health resources for travelling older adults, our team has developed a web application that seeks to: Create a single place to make quick medical decisions and get access to health information in a SNAP. 3www.mysnaphealth.com

Background Research There is currently no site for winter travelers to access local health information. Current techniques for gathering information include – Calling home health agencies – Using the phone book – Getting friend referrals An article from the American Medical Association describes the way doctors have to get emergency information through Fax! Information only available during working hours Patients carry info in wallet on crumpled piece of paper No quick way to access basic information – –

Potential for Impact SNAP Health will use data from HHS and other sources to help these visitors make informed decisions about seeking care in their vacation state and store some basic health information for quick access in case of emergency. HHS Data Used – 30 day mortality rates for Pneunomia, Heart Failure, and Heart Attacks by state. Other data used – Data.Medicare.gov– Patient overall satisfaction ratings by Arizona hospital – Google Maps/Google Ratings

App Design and Usability End user considerations – Easy site navigation (Steps 1-2-3) – Generated how-to video – Simple graphical interpretation – Easy mobile access – Quick access to personal health data Data used – HHS data for mortality rates in hospitals in Arizona, Texas, and Florida – Medicare data for patient satisfaction on a per hospital basis in Arizona – Google user ratings for hospital locations

Creativity and Innovation Easy introduction video to help the user understand how to navigate the site efficiently Location-Where are you going? – Arizona Hospitals, User Ratings, Patient Feedback – Florida, Texas Coming soon! User Profile-Chart your health. – Emergency Contacts, Primary Care Physician, Allergy info, Medications, Medical History and Bloodtype 7www.mysnaphealth.com

Next Generation and Scalability Designed so that when the patient is heading to another area, they can select which healthcare providers have permission to access the patient’s information. The healthcare facilities will have information on what insurance is accepted to help aid the patient in their decision of where to receive care. Links to: -Travel/booking sites -Car rentals -Local coupon deals. Store legal documents for quick access by medical professionals in an emergency Background login for practitioner so that they can see information that the patient has marked as public. Temporarily store medical records for travelers in case of emergency.

Sustainability and Use of Funds Continued website development Mobile app development Develop a pay for service side of the app (travel deals, etc.) Possible partnerships with – Pharmacies – Snowbird associations – Healthcare providers Widen demographic – College students Living in a new location Unfamiliar with health environment and where to seek care

Summary of Impact SNAP Health SNAP Health can impact the following healthcare outcomes: 1.Decrease time required to access basic emergency health information for people outside of their local health resource area. 2.Provide vetted information about hospitals and other health resources in popular long stay vacation states using HHS and other data sources. 3.Fill the gap between a full personal health record and a hospital based Electronic Medical record. Easy to access basic health information!
