Individualized Service Plan

Who attends the ISP?

-Children - Families -DHR worker -Other individuals requested by the family

What does an ISP contain?

Basic health, mental health and education needs. Additional assessments needed to identify strengths and needs. Steps to meet the child’s safety needs.

Attachment needs. Desired outcome for the children and time frame to achieve that goal. Steps to promote a timely return home or placement with relatives if part of permanency goal.

When will I receive a copy of the ISP?

A written copy should be provided to age- appropriate children, parents, and team members at the conclusion of the ISP meeting, or within 10 working days of the date the meeting was held.

When is the ISP reviewed?

As necessary Initial ISPs will be reviewed within 30 days of the initial ISP At lease every 6 months
