3 PUBLIC HEALTHCARE AGENCY Barcelona – PRIMARY HEALTHCARE CENTER El Carmel Public Healthcare Agency of Barcelona (ASPB) Public organisation in charge of monitoring health services, promotion and prevention for all the population in Barcelona. Primary Healthcare Centre (CAP) The first access of the citizen to the public health system. Promotion, prevention, attention and rehabilitation of health in a defined neighbourhood.

4 Working team Multidisciplinary team that includes: 2 nurses of the ASPB and various professionals of the CAP including administrative personnel, social worker, nurses and doctors.

5 Projects ­Memory training workshops ­First aid and basic CPR ­Physical activity community group ­Conjunct scholar vaccination between PHS and PC. ­Common participation in the community plan All the projects are community oriented

6 Memory trainning workshops Avoid and delay the neurological deterioration in population over 60 years old through memory stimulation exercises, improving the independence and quality of life ­Memory ­Self-esteem ­Physical exercise ­Nutrition Activity: 5 workshops. 13 sessions of 1h30m. 60 participants.

7 First aid and basic CPR workshop for students Approach to appropriate actions in front of emergency situations and accidents following the red cross international protocols (Protect, Alert, Help). Other subjects treated in the workshop were frequent health problems and their solutions, without forgetting the healthcare kit indispensable for first aid. Activity : 3 workshops. 3 hours duration each. 75 participants.

8 Physical activity and promotion health community group Promote the creation of groups formed by people < 60 years to realize group physical activity independently with support from the center. Health education sessions based on subjects decided by the group. Activity: 5 groups. 2 hour per week for 11 months. 80 participants.

9 Conjunct scholar vaccination Collaborate work between a nurse of the CAP and nurse of the ASPB: ­Share previous experience and prepare the new work model. ­Td vaccination of all the 14 years old children of all the schools of our neighbourhood. ­Prepare for the school year to complete the vaccination program ­Realize all the school vaccination with the new model.

10 Common participation in the community plan Community Plan Carmel Amunt CAP Carmel and the ASPB participate in work groups with all the neighbourhood associations and all the neighbours: Health promotion activities. Healthy alimentation games for children Radio program Food recollection campaign

12 Conclusions Combined action between CAP and ASPB in the community health plan. ­Facilitate ­Complement ­Potentiate Conjunct work increases the benefits for the community. Working together takes more time to achieve consensus, but the results obtained are more effective
