Distribution of Folders with Documents ◦ ACIS signature page – June 23 due  Only needed if it was in packet; most already submitted ◦ Trip Permission slip for HF-L - June 23 due ◦ Behavior Contract – June 23 due ◦ Medications Permission Slip  How this is handled is your physician’s decision  Some docs one form per trip; some docs multiple trips per form  To be discussed at physical time / each doctor is different  Required by May 2015 ◦ Scholarship Contest ACIS Attendance and Introductions
