WALKING THROUGH THE QUARTERLY CONTACTS REPORTING PROCESS

THE QUARTERLY CONTACTS SUMMARY REPORT

The Quarterly Contacts Summary Report is a summary of the contacts tracked for each quarter based on the following categories: Calls/ s Home Visits IEP Meetings Mass s/Bulk Mail/Newsletters Parent Meetings # of Parent Trainings # of Parents Trained # of Parent Trainings Using a P2P Presenter # of Teachers/Administrators Trainings # of Teachers/Administrators Trained # of Other Events # of Attendees at Other Events

Contacts Tracking Explanations Calls/ s These are calls to and from a parent. Each call you take or make is counted. Each you send and receive is counted. Each text you send and receive is counted. Home Visits Each home visit is counted as one. IEP Meetings Each IEP Meeting is counted as one. Mass s / Bulk Mail/ Newsletter Insert the number of mass s or bulk mail sent out during that month. Example 1: If you send out a mass to 150 recipients, put down 150. Same for newsletters (# you sent out) Example 2: If you send out 2 mass s to 150 recipients, put down 300. Parent Meetings These are one-on-one meetings/consultations with parents.

Contacts Tracking Explanations Continued # of Parent Trainings Here you will count the number of trainings you have hosted or presented (your own and with another organization) # of Parent Trained Here you will include the # of parents who attended the trainings (including your own and with any other organization) # of Parent Trainings using a P2P Presenter Here you will include the number of parent trainings that you hosted using P2P. # of Teacher/Administrator Trainings Here you include the # of presentations done at Department, school, or county office level meetings.

Contacts Tracking Explanations Continued # of Teachers/ Administrators Trained Here you will include the number of teachers or administrators present at the presentation. # of Other Events Any other event or meeting where you wore your "Parent Mentor Hat“. # of Attendees at Other Here you include the number of people who attended the other events. Example: 3 events with 5 people at each =15

Each quarter consists of three months : 1 st Quarter: July/August/September 2 nd Quarter: October/November/December 3 rd Quarter: January/February/March 4 th Quarter: April/May/June

Quarterly contacts are due on the 15 th of the month following the quarter that is being reported on. 1 st Quarter: report due on October 15 2 nd Quarter: report due January 15 3 rd Quarter: report due on April 15 4 th Quarter: report due on 7/15 or before you leave school

Quarterly Contacts are to be submitted into Google Docs using a Google Docs link. Report your quarterly contacts at the link that will be sent to you by your Regional Rep via . Or log in to the Learning Curve. To log in use the following: User id: partnership Password: mentor Then click on Accountability – Annual Reports (first link on the right), scroll down to ”Your Region Name” Link to Quarterly Contact Reporting. Click on Your Region Name.

You will need to submit the totals for each category for the three months of the quarter that you are reporting data on.

1 ST QUARTER QUARTERLY CONTACTS REPORT

For the 1 st Quarter Report: July/August/September You will be submitting the totals for each category for these 3 months

You will need to submit the 1 st Quarter Quarterly Contacts Report by Thursday, October 15, 2015

INSTRUCTIONS

STEP 1: ADD the totals for each category for the months of July 2015, August 2015 and September 2015 Example: Calls/ s July - 10; August - 5; September – 8 Total Calls/ s: = 23

STEP 2: ENTER the data into the appropriate boxes in the Google Docs Link. Example: Calls and individual s * Each call to and from a parent is counted. Each you send or receive is counted. This is a required question 23

EXAMPLE: WHAT YOU SHOULD SEE INSIDE THE GOOGLE DOCS LINK FY15 - Middle Georgia Contact Reporting Please enter the quarterly contacts onto the form below. If there is anything that you need to explain, use the last question to do so. Enter a "0" for any category that you do not have numbers (ex. if you don't do home visits, enter 0) Hopefully this will make contact reporting MUCH easier to report contact ON TIME. If you have any questions, contact your regional rep before the due dates: Oct 15th, Jan 15th, April 15th and July 15th (or before you leave for the summer) Allison Stevenson

STEP 3: HIT the SUBMIT Button SUBMIT

You'll see a Thank You message on screen when successfully submitted.

In the event that you experience technical difficulty, or the submit button is not working, please contact your regional representative before the October 15 th due date for an alternative submission method so that you will receive credit for turning in your report by the designated due date.
