Long-term Care Facilities provide assistance and care for elderly patients (residents), people with disabilities, and people with chronic or long-term illness some called nursing homes: provide care when patients can no longer care for selves. some called extended care facilities: nursing and rehab care to prepare patients to return home or to other facilities. some called independent living/assisted living: provide assistance to patients who can care for themselves.
