Who is referred and how? Mild to moderate health conditions –Anxiety –Depression –Arthritis –Back pain –Cardiac conditions –‘Other’ (gout, fibromyalgia, ME, diabetes, and more!)

Who is referred and how? Referrals –From IB Personal Adviser or Disability Employment Adviser –Claiming IB or Employment Support Allowance –New claimants –Can volunteer into Pathways if on Incapacity Benefit

What does the team do? Understanding and coping with a health condition Not diagnosis or treatment Allow the person to have more control over their health and work Help the person achieve their goals of getting back to work, now or in the future

What does the team really do? Every programme is individual Maximum 14 sessions Practical and focused on personal goals 1:1 and CMP courses Cognitive behavioural approach Assist to link in with local activities and resources
