What is a FHT? “An approach to primary health care that brings together different health care providers to co- ordinate the highest quality of care for the patient ” MOHLTC Collaboration between providers Extended hours Telehealth Catchment area

Newcomer health Newcomers < 5yrs face many barriers in accessing health care and social services Barriers: long wait times, language, financial, childcare, lack of info, travel Why focus on young women and children? FHT mandate: local community (43% immigrants as per 2005 census data) MOH mandate: unattached, complex, marginalized people with difficulty accessing system

YIW & their children Initiative Women less than 45 yrs of age, children In Canada less than 5 years OHIP or other (i.e. Interim Federal Health) Comprehensive care Language Line/Interpretation services Intake
